CCHA’s Healthy Rewards Program allows you to earn reward dollars for completing healthy activities like attending health care appointments. Healthy Rewards helps you stay motivated and empowered to make healthy choices and stay up to date with important personal health milestones.
It's simple. Enroll in the healthy rewards program and request a gift card of your choice to receive when you complete healthy activities.
To enroll
- Visit the website benefitrewardhub.com/CO or call 1-888-990-8681 (TTY 711)
- Activate your account using your Medicaid ID as your subscriber ID, along with your name and date of birth. You can easily enter your date of birth from the pop-up calendar during registration.
- If you are activating an account for your child, use your child’s name, date of birth and Medicaid ID
